We continually look for ways to improve the accessibility of the service, including helping applicants to supply as much information as required to complete the application for a check. We also understand that sometimes the information won’t all fit on the form.
So, if applicants do find that they run out of space when recording all of the necessary information, please remember to advise them to use an official CRB continuation sheet.

The continuation sheet includes multiple fields to help applicants record any relevant information that will not fit on the application form itself.
The sheet, which is double sided, is designed to be submitted with the application form. It also includes fields at the top of each page to record the form reference number so that we can link the sheet to the right application form.
