Avoid delays when completing a Basic Disclosure Application

If you’re looking to avoid delays in receiving your Basic Disclosure, make sure to follow these simple steps when completing your Basic Disclosure application form.

 

  • When completing a Basic Disclosure application form, all yellow fields must be completed. However, if you complete all other boxes and provide as much information as you can, it may speed up your application

 

  • Remember to send 3 ID documents back with the application form (just photocopies) – within the 3 documents you provide, you must prove your date of birth and current address

 

  • Middle Names – in section the forenames always remember to enter any middle names you have. Failure to do this may delay the processing of your Basic Disclosure application

Proof of Identity – what do you need to provide?

Basic Applications

You need to provide photocopy proof of identity with all basic applications. 2 pieces of identity are required:

1.  Proof of date of birth (birth certificate, passport or driving licence)

2.  Proof of current address (bank statement, utility bill or council tax statement)

Standard, Enhanced or Volunteer Applications

You do not need to send any photocopies of ID documents with these applications as the verifier completes Section W, and by crossing the “yes” box in W59 they confirm that they have seen enough valid and original ID

Addresses on a Basic Disclosure

Basic Disclosure Address Information

Recently there has been an increase in errors with addresses on a Basic Disclosure forms – if you are completing these forms please ensure that you complete the year moved into your current address and also any previous addresses, you must provide a 5 year address history even if you have lived outside of the UK and include the year moved into each address.

How to Complete a CRB Form

Important information for a verifier on how to complete a CRB Form correctly.

Once your applicant has completed Sections A, B, C and E of the CRB Check the verifier needs to complete the following:

Section W

The name of the nominated verifier must be entered in W58 and they must see enough valid and original id for each applicant before they cross the “Yes” box in W59.

By completing Section W the verifier is confirming to us that they have seen and checked enough valid and original ID.  Please do not send photocopies of id or original documents.

Section X

The verifier must also complete X61, X62 and X67.

Please do not change or amend a job title, titles such as “Director” or “”Partner” are not recognised by the CRB as they are not specific enough.  You must be more specific ie “Director of Training Company”.

Section Y

Please leave this blank as this is where Personnel Checks sign, the verifier does not sign the application form as their details are in Section W.

Section Z

Please leave this blank as the CRB complete this section.

CRB Form Errors on Standard and Enhanced Applications

CRB Form Errors and How to Avoid Them

Incorrect CRB forms cannot be processed and will delay your application.

To avoid this please ensure you read our guidance notes a couple of times before attempting to complete your application form, they are designed to help you.

Standard/Enhanced Forms

Applicant not signing in Section E – your form is automatically rejected and returned to you.

Applicant signing in blue ink – your form is automatically rejected and returned to you.

Tippex used on forms is not permissible and your form will be rejected.

Section W – your verifier must complete W58 and W59.

Incorrect Spellings

Names that are spelt incorrectly cannot be corrected once a form is sent for processing, it is the responsibility of the applicant to ensure their form is correct before they sign Section E.

The verifier must also check that the name of the applicant, job title and company names are all correct before completing Section W.

Basic Disclosure Application Form Errors

How to Avoid Basic Disclosure Application Form Errors

We have received a lot of incorrect Basic Disclosure Application Forms this month, incorrect forms cannot be processed and will delay your application.

To avoid this please ensure you read our guidance notes a couple of times before attempting to complete your application form, they are designed to help you.

Common Basic Disclosure Application Form Errors

B5 – Mothers Maiden/Family name – we require the surname only

B6 – previous names used – this includes your maiden name and any other previous names you have used

Year moved into each address must be included

Address History must go back 5 years

Incorrect Spellings

Names that are spelt incorrectly cannot be corrected once a form is sent for processing, it is the responsibility of the applicant to ensure their form is correct before they sign Section E.

The verifier must also check that the name of the applicant, job title and company names are all correct before completing Section W.

How can I get a quick CRB Check?

To avoid mistakes and errors which will delay your application, we strongly encourage you to take a couple of minutes to read through the guidance notes which are enclosed with the application forms.

Forms must be completed in black ink and in CAPITAL letters, each letter should be clearly defined. Leave a blank square between words but not between dates.

The following errors will delay your application:

  • Forms completed in blue ink – these are instantly rejected as only forms completed in black ink can be processed.
  • Tippex on forms – these are instantly rejected as they cannot be sent for processing. Cross any errors out neatly and write the correct answer close to the relevant question.
  • Forms signed in blue ink – again these are instantly rejected and means that we will have to send your form back to you to re-sign in black ink.
  • Name is clear and correct – your applicant signs their application to say that all information is correct. If their name is spelt incorrectly and the form is sent for processing, the incorrect name will be printed onto the disclosure and cannot be corrected. You would have to re-apply and pay again for a new check if this happens.
  • Middle name is missing – many people have a middle name but tend not to use it, this usually shows up when we try and match the driving licence number with forenames. Even if you do not use your middle name you must supply this information on your application. Don’t forget to include your middle names with any previous names you may have used.
  • “Mrs” or “Ms” – If your title is “Mrs” or “Ms” please ensure that we have all previous surnames and forenames that you have used, together with the month and year (MM/YYYY used from and to.
  • Incorrect Postcode: If your postcode is incorrect then your completed disclosure may be delivered to an incorrect address. Disclosures contain personal, confidential information so it is important your full postal address is correct.
  • Crossing out sections of form – please do not cross out any section of the form or write “n/a”, if a section does not apply leave it blank.

Signing a CRB Application Form

To ensure the forms are not rejected it is essential that the white box on the back of the CRB Application form is signed by Personnel Checks – not the evidence checker.

Please ensure that the evidence checkers appointed by your organisation are aware that they should never enter their signature in the box, as this will only lead to a delay in processing the CRB Application.

Section B of the Basic CRB Application Form

Basic CRB Applications- Additional Information Section

When completing the “Additional Information” section on a Basic application, you need only complete one of the fields from B19 – B25. The most popular field to complete is B19 your National Insurance Number.

Section Y of the CRB Application Form

Personnel Checks have noticed that a lot of customers answer section ‘Y’ on the CRB application form which is located on the back page. Please remember that this is for us as the Umbrella Body for the CRB to complete, not the organisation.

The only parts of the application form that the verifier of ID at the company needs to complete are the ‘Registered Body Use Only’ boxes, which are in sections A & B. Please thoroughly read through the ‘Verifier Guidance Notes’ we provide to ensure you fill in the correct boxes.

Please take the time to complete this part of the form with care in order to avoid delays in the CRB application process.

Full Name on a CRB Application Form

In order to assure the CRB is able to check your identity, you must provide your full name on a CRB Application Form.

If you do not provide your full name, including any middle names and previous names you may have used, this could affect the CRB’s capability to correctly identify you. The accuracy of our service relies on the information provided by the applicant and if all available information is not provided, this may result in an inaccurate CRB certificate.

In section A of the application form, we ask the applicant to provide their current name and verify if they have been known by any other names. You must ensure that, if the applicant ticks the YES box at a4, they provide their previous names in full; i.e. that with each surname provided, the applicant has also provided all forename(s) they used with that surname. We are unable to assume that their forename has remained the same and if this information is omitted, we will reject the application form.

The dates between which each of the name combinations were used must also be provided.

It is your responsibility to ensure that this section of the application form is completed correctly and that the applicant’s identity has been established before you submit the form to the CRB for processing. If any parts of this section are incomplete, the application form will be rejected and returned to you to clarify the details with the applicant, which will add time to the overall process

Section A in CRB Application Form

Help with A1 to A4 of the CRB Application Form – This is an area where common errors occur.

A1 – Applicant title, all applicants have a title, if they have completed “Mrs” or “Ms” then we need to know the other surnames and forenames they have used (even if they do not use them now). If an applicant chooses to call themselves “Ms” or they did not change their name when they married we need to be advised of this on a separate piece of paper.

A2 – Surname please make sure that your applicant fills in their surname first!

A3 – Forenames please complete all forenames including middle names even if you do not use them we still need to know. Your middle name is usually shown on your driving licence and if you provide this as id then it must match with the forenames on your application

A4 – Any other names? If you cross “Yes” here then you must complete the next section A5 – A6 and A7 – this requires the month and year you used your previous names from and to

If an applicant uses their married name as well as their maiden name for business purposes we need to be advised of this.

Make sure your name is spelt correctly and clearly as this is how it will appear on your completed disclosure.

If you hold a valid passport of any nationality then A24 should be crossed as “Yes” and the following fields must also be completed:

  • A25 – Passport Number
  • A26 – Nationality
  • A27 – Country of Issue

If you do not have a valid passport please cross “No” and leave A25 – A27 blank. Some applicants have been crossing the “No” box and then writing in their nationality – your CRB Application form will be rejected if you do this.

Black Ink is a Must on CRB Check Application Forms

We have had an increased amount of forms coming back to us where the applicants have used blue ink on their application forms.

This means that the form will be immediately rejected, and we will have to send out another application form to you. This can delay the process by a few days, therefore please remember that only black ink is accepted on the application forms.

Correct Spelling on CRB Check Application Forms

On the front of each CRB Check application form, you are reminded to write clearly, using black ink and capital letters. This is to ensure that the information provided is clearly recognised and that your applications are processed as quickly as possible, preventing any further requests for clarification.

In addition, it is important that spelling on a CRB Check Application Form is correct. This is particularly important for the name, address and position applied for fields of the application form. If a word is spelt incorrectly, it can impact on the whole application process, adding time and often inconvenience for all parties involved. Specifically for names and addresses, we will often need to clarify at a later stage in the process if the detail that has been provided is correct.

In the ‘position applied for’ field, x61, if a word is spelt incorrectly or is unclear, the position may be misinterpreted and may appear to be ineligible. This also applies to abbreviated job titles, which will not be understood outside of your organisation. In addition, for home based occupations, an incorrect spelling could impact upon the additional checks carried out by the police.

Please pay attention to these points when checking that the applicant sections have been completed correctly, and when completing your own sections and countersigning the application form. We would encourage you to check that the information provided is clear and double check words if you are unsure of the correct spelling, before submitting the application to the CRB.

CRB Application Errors

Here are common CRB Application Errors, avoid these when completing your CRB form.

1. Verifier boxes not crossed (either section a, b or w or all 3)

Verifiers are confused by these boxes as they say “registered body use only” but in this case you are acting for the registered body as they are checking all original id.

Section A “registered body use only” box, cross A1 – A3 + A14 if you have seen the applicants birth certificate, cross A21 if you have seen their NI Number, cross A23 if you have seen their Driving Licence number and A25 for their passport.

Section B “registered body use only” box – cross this box to confirm that you have seen something official with their home address on, i.e a Bank Statement, Utility Bill, Council Tax Statement – we do not need to know what you have seen unless you are counting this as proof of id.

Section W – the verifier name goes in W58 and “yes” is crossed in W59 to confirm that all id seen has been original.

Remember to check you have seen enough ID and if the ID you have seen does not fit onto the form you will need to complete and return the ID continuation sheet with your application.

2. Previous Names used (applicants who have put Mrs or Ms but no other name)

We need to know all previous surnames and forenames used together with the month and year used from and to

3. Address history incomplete (either address history not back to 01/2006 or month/year moved in missing)

A full 5 year address history is required – we need the month as well as the year applicants moved in and out of addresses

4. Not enough ID presented (enough id has probably been seen but not communicated to us)

Check your list of acceptable ID documents

5. Driving Licence queries (wrong number given or letters/numbers missed)

This begins with the first 5 letters of the applicants surname, followed by 6 numbers of their date of birth which are muddled up, followed by either 1 or 2 letters (these letters are the applicants initials and if there is a second letter it shows they have a middle name), followed by computer generated numbers and letters.

We can accept a provisional driving  licence as proof of id

Top 5 CRB Application Form Errors

Personnel Checks receive thousands of completed CRB application forms every week. Many applications we receive have not been completed properly by the applicant which results in a delay in the application process. Take a look at our top 5 list of common CRB application form errors to avoid in order to keep your CRB application on track.

  1. The Month that applicant moved into address is missing – address fields need the month and year completing in MM/YYYY format
  2. Applicants with the title of “Mrs” or “Ms” NEED to include previous surnames and all forenames used
  3. Verifier box in Section B MUST be crossed to confirm that current address has been verified
  4. Unsigned applications – we cannot process these and they will be returned to you
  5. Applications that are signed in blue ink are automatically rejected as the CRB will not accept blue ink anywhere on the form

Common CRB Application Errors

Here is a list of the common CRB Application Errors reported by the CRB for the month of May 2011.

In order to process your CRB Checks as quick as possible, try not to make these mistakes as incorrect CRB Application Forms will be returned to you for corrections.

Addresses on a CRB Check Application Form- Student Addresses

Student Addresses

While it is common for an applicant to live on campus or in other student accommodation during term time but then reside with their parents (or a similar permanent home address) in the period between academic years, this can cause confusion when completing the current and previous address fields on the application form.

We have therefore summarised some key guidance points below to help you to relay the correct information to your applicants. The address entered in section b – current address, should always be the address where correspondence can be sent to the applicant and where the certificate will be sent to at the end of the process.

Parents’ address If this is the applicant’s parents’ address (or similar permanent home address), the date the applicant has been ‘at the address since’ in b37 should be the month and full year they originally moved into that address.

If the applicant has lived at this address for at least five years, the applicant must still provide details of all the student addresses they have lived at during the last five years in section c – other addresses, so that we have a full address history for that period.

As it is likely that there will be gaps between student addresses, between academic years, we will use the applicant’s parents’ address (or the similar permanent address supplied) to fill in the gaps.

If the applicant moved into their parents’ address less than five years ago, section c must include any previous family homes where the applicant has lived outside term-time during the last five years. The applicant must inform us, on a continuation sheet, as to which of the addresses provided are, or have been, the main permanent residences. This information must not be written on the application form itself.

Student address If the applicant prefers that we contact them and send their certificate to their student address, this should be provided at section b – current address and their parents’ address (or similar permanent address) provided in section c – other addresses.

The full address details of the halls of residence or other student accommodation lived at in the last five years must be recorded in section c. The only field that is not mandatory is the ‘county’ field.

If the applicant is still living at their most recent student address, the ‘dates from and to’ fields must be completed, but the ‘date to’ will be the month and year in which the applicant completes and signs the application form. This field must not be left blank or the application will be delayed.

Similarly, if the applicant’s parents’ address (or similar permanent address) where the applicant resides outside term-time has been provided in section c, the ‘date to’ will be the month and year that the applicant was last residing at this address.

The words ‘current’, ‘still there’, present’ or any similar phrases must not be recorded in any of the fields or anywhere else on the application form.

The form allows space for two additional addresses; any further additional addresses must be supplied on an official continuation sheet which can be downloaded from www.homeoffice.gov.uk/crb-continuation-sheet.

Addresses on a CRB Check Application Form – Unusual Addresses

Unusual Addresses When Filling out a CRB Check Application

We will clarify how to complete the application form if the applicant is working, or has worked away from home during the last five years or if the applicant is a member of the Armed Forces.

Working away from home

  • We do not need the details of addresses other than an applicant’s permanent residence: If at any time in the last five years they have been required to spend a day or two away from their permanent address due to their work. In these situations we do not need the details of hotels or similar types of residence they stayed in.
  • If the nature of their work, for example, a travelling salesman; locum; supply teacher; pastor; or speaker means that they are frequently spending one or two days in different places away from their permanent address. We do not require the details of each place they visited.

We do need you to provide the applicant’s alternative address details as well as the details of their permanent address:

  • If they spend months at a time living at an alternative address that is not their permanent home address, we require the details of this residence and the dates between which they were living at the alternative address to be entered in section C.
  • If their work requires them to reside somewhere other than their home address during the week but they return to their permanent residence at weekends.

How we need you to complete the form:

  • Ensure the applicant has used section B to provide the details of their permanent home address – and that they know we will use this address to write to them and send their CRB certificate to.
  • Ensure that they use section C to tell us about the other address they use on a frequent basis.
  • We require the month and year at which they first started to stay at that address and the month and year in which they last stayed at that address. NB. The dates ‘from and to’ must be completed, so if the applicant continues to use this alternative address at the time of their application, please provide the current month and full year as the ‘date to’.

HM Armed Forces

  • If the applicant is or has been a member of Her Majesty’s Armed Forces and they have a British Forces Post Office (BFPO) number, they may use this as their current address in section B of the form or, if relevant, as part of their five-year address history in section C.
  • Please be aware that due to a specific arrangement with HM Forces, the fields that are mandatory for all other forms of address are not required for BFPO addresses. The applicant can write their BFPO number in the ‘address’ field and leave the town/city, county, UK postcode and country fields blank.
  • If the applicant supplies the BFPO number as their current address, we still require the month and full year at which they began using this address in B37.
  • If the applicant is supplying the BFPO number as part of their ‘other addresses’ history in section C, we still require the ‘dates from and to’ that they used this address

Addresses on a CRB Check Application Form – ‘No Fixed Abode’

We cannot accept ‘NO FIXED ABODE’ as the applicant’s current address in section b.

We cannot accept this on a CRB Check Application Form as we require a full address at which we can write to the applicant and send their copy of the certificate.

In these circumstances, the applicant must provide a ‘care of’ address in section b and provide an explanation on a continuation sheet which can be downloaded from www.homeoffice.gov.uk/crb-continuation-sheet

Driving Licence Details on a CRB Check

The Criminal Records Bureau often find that they are given incorrect Driving Licence details on a CRB application form. To help applicants to complete this section correctly please note the following:

A UK driving licence is made up of numbers and letters generated in accordance with the licence holder’s personal details.

If the driving licence number indicates a middle name but the applicant has not included it on the application form, a letter will be sent from the CRB to ourselves, Personnel Checks to explain the discrepancy and request clarification. Until clarification is received, we will not be able to progress the application any further.

Last month, the CRB received 2174 application forms with errors relating to the middle name characters in the Driving Licence number. Please remember to carefully check the form before countersigning and sending it to the CRB.

Documents Needed for CRB Check – Standard & Enhanced Applications

Please note that for the Standard and Enhanced CRB Checks, we do not need to see any photocopies or original documents, they are just for you to see and verify.

It is vital that as the evidence checker you:

  • You must only accept valid, current and original documentation.
  • You must not accept photocopies.
  • You must not accept documentation printed from the internet e.g. internet bank statements.
  • Identity information for the applicant’s name, date of birth and address recorded in Section A and Section B on the CRB application form must be validated.
  • You should in the first instance, seek documents with photographic identity (e.g. passport, new style driving licence, etc.) and for this to be compared against the applicant’s likeness.
  • All documents must be in the applicant’s current name as recorded in Section A (see below for guidance on recent changes of name).
  • One document must confirm the applicant’s date of birth as recorded in Section A.
  • You must ensure that the applicant declares all previous change of name, and provides documentary proof to support the change of name.
  • You must see at least one document to confirm the applicant’s current address as recorded in Section B, which is no older than three months.
  • You must provide a full and continuous address history covering the last five years. Where possible you should seek documentation to confirm this address history.
  • You should cross-match the applicant’s address history with any other information you have been provided with as part of the recruitment, such as their CV. This can highlight if an address has not been given e.g. if the applicant’s CV shows that they have worked in Liverpool in the last five years but the application form only shows London addresses, you may wish to question the applicant further about this.
  • A document from each of the groups should be included only once in the document count e.g. do not accept two bank statements as two of the required documents, if they are from the same bank.
  • You should not accept the foreign equivalent of an identity document if that document is listed as ‘(UK)’ on the list of valid identity documents.

ID Verification for CRB Check

Make sure ID Verification Has Been Carried Out Correctly

One of the most common mistakes on the CRB application forms is the verification of the applicants’ ID – The ‘Registered Body Use Only’ boxes in sections A and B do not get crossed which can delay the application.

These boxes are for the designated person we chose within your company to cross, to confirm what ID documents you have seen from the applicant.

The boxes correspond with the questions on the left, for example, the first box to cross says A1-A3 verified, which on the left are the questions Title, Surname and Forename – once you have seen a piece of ID that confirms all three of those, you cross the box.

Required ID for a CRB Check

From 28 May 2012, the CRB will be introducing enhancements to identity checking guidelines. These new guidelines will apply to required ID for a CRB Check.

The enhancement is being introduced so that the CRB’s identity checking process is strengthened to improve public protection. In particular, the changes will make it more difficult for individuals to conceal previous criminal records by changing their name. These changes are part of an on-going improvement process that will enable easier detection of undeclared changes of name in the future.

The new developments in the required ID for a CRB Check will have three routes. Please note that some documents have been removed and are no longer acceptable under the new arrangements.

The new guidelines clearly state that you need to follow the three-route ID checking process. ALL Customers must have exhausted Route One and should have endeavored to have accessed an external validation check (Route Two) before you consider processing them via Route Three.

Avoid Common Errors when completing a CRB Form

Avoiding Common Errors when completing a CRB form will speed up the processing of your application:

Before completing a CRB form please read the guidance notes that were enclosed with your application form. This will help you to complete the application correctly and to avoid common errors which will delay the processing.

Basic CRB Check Disclosures

The Top 3 errors made by applicants completing a Basic Disclosure Scotland Forms are:

  • B5 – Mothers Maiden/Family Surname is missing
  • B6 – Previous names are missing – you must include all previous surnames and forenames that you have used, ie; if you have married and changed your surname then your maiden name would be entered here
  • B35 – Resident From – this requires the Year that you moved into your address. Your address history must go back 5 years. If you have lived at other addresses we need to know the year you moved into these addresses as well

Standard and Enhanced CRB Check Applications

Top 3 Errors when completing a CRB form for Standard and Enhanced Applications are:

  • E56 - Applicant not signed application or has signed in blue ink – this results in the application being returned as we cannot process without a signature and blue ink is not permissible on the application
  • A4 – A7: Previous names are missing. If you changed your name when you got married please ensure that your maiden name is written in these fields together with the month and year this was used from and to
  • Y72: Verifiers are signing this declaration box – this must be kept blank as this is where we sign as the registered body. Verifiers are not required to sign the application form, their details are just entered into Section W.

Volunteer CRB Checks

When completing a CRB form, the part of the application that regularly features at the top of the ‘Top Ten Errors List’ is; ‘is the application for a free of charge volunteer?’

The CRB defines a volunteer as ‘a person who performs any activity which involves spending time, unpaid (except for travelling and other approved out-of-pocket expenses), doing something which aims to benefit someone (individuals or groups) other than or in addition to close relatives’.

For the CRB check to be processed free of charge, you must ensure that:

  • The position applied for meets the criteria of a volunteer as defined above
  • The position is eligible for a CRB Check
  • Question X61 describes the nature of the position – do not just use the word ‘volunteer’
  • You cross (X) the ‘Yes’ box in Section X68 of the application form to indicate that it is for a free volunteer check

When completing a CRB form, if you fail to indicate that you require a check for a volunteer by leaving this question blank or if you tick both boxes in error, the form will be returned to you and you will be asked to clarify before the application can be processed any further. If the application is submitted for a volunteer and a cross (X) is placed in the ‘Yes’ box, but a payment is received, this will also be rejected and returned to you.

There are also circumstances when question X68 is completed ‘Yes’ and the position is in fact not for a volunteer position. We perform regular audits on applications for volunteers to ensure compliance with the criteria for ‘free of charge’ checks. If Section X68 is completed in error and does not concur with the ‘position applied for’, we will contact you to draw your attention to the error and will make a retrospective charge to recover the application fee.

CRB Application Form Top 10 Errors

Below are the top 10 reasons why CRB Application forms are rejected, resulting in the process being delayed. If you avoid these errors, we should be able to return your completed CRB check quickly and easily.

1. Blue ink or Tippex has been used on the CRB Application Form

2. Applicants have not signed in Section E

3. Verifiers have not crossed “Registered Body” boxes in Sections A and B to confirm the id they have seen

4. Address History is incomplete – this must go back 5 years and include the month and year moved in and out

5. Applicants with the title “Mrs” or “Ms” who do not complete previous names and month and year they were used from and to

6. Incorrect Driving Licence numbers – only UK licences are acceptable and these begin with the first 5 letters of the applicants surname, followed by 6 numbers of the date of birth, followed by either 1 or 2 letters for Forename and Middle name, followed by computer generated numbers/letters

7. Middle name is missing – applicants must state this even if they don’t use it

8. Job title is missing from X61

9. Are applicants new, existing or recheck postholders re: X67

10. CRB Check forms not completed in CAPITAL letters or illegible numbers

Please remember to leave section Y on the CRB Application Form blank. We appreciate it’s really tempting to sign in the box provided but this is for Personnel Checks to sign as the registered Umbrella Body.

The only signature required on the CRB Check form is the Applicants – Thanks for your assistance.
